Hi All!

Do you want to know when events are happening in your area, or around the world? Do you want your kids to come to events? Do you want to kept up on what's happening in Subud?

For us to let you all know these things, we need some information to reach you more effectively.

Here's what we would love to learn about you:
1. Your name (past first and last names are equally useful)
2. Your mailing address
3. Your best and most current email address
4. Your phone number (optional)
5. If you have children, your children's names and birthdates. (If you are under 18 please ask your parents for permission before giving me any info.)
6. Your birthdate
7. Is it ok to give your info to the US nat'l office database?- (if you live in the US)
8. If you live in another country, we would love your email addresses so you can be added to our e-newsletters (info on US events, discussions, news, announcements, etc.)
9. Your Skype address
